Mayer von Rokitansky Kuster Hauser syndrome is a uterine-vaginal malformation with normal development of secondary sexual characteristics. Diagnosis is often made during adolescence, with the cardinal symptom being the absence of menstruation. However, cyclical pelvic pain is another, albeit rare, reason for consultation. We report the case of a 13-year-old adolescent who complained of cyclical pain located in the hypogastrium and pelvic region. Based on the physical examination findings, an ultrasound was performed, revealing a functioning Müllerian derivative and an ectopic kidney, and she was taken to the operating table. Knowledge of the syndrome and its variants is essential for accurate diagnosis and proper management, due to the implications for the physical and emotional health of the patient and her family.
