Pescado: un cuerpo extraño infrecuente en pediatría. Reporte de un caso

Abstract

Introducción: En la alta Goajira del estado Zulia, Paraguaipoa, la pesca es un oficio que practica la gran mayoría de la población, siendo el sustento económico y alimentario de las familias de la zona. Por lo insólito, se reporta el caso de un adolescente de 14 años quien fue traído a la Emergencia de Pediatría del Servicio Autónomo Hospital Universitario de Maracaibo, por ingerir en forma accidental un pez, mientras estaba pescando con una red, el cual salta y cae en la boca del paciente, presentando náuseas, angustia, dificultad para respirar, salivación excesiva, orofaringe eritematosa; a la laringoscopia directa se observa un pescado que ocluye la orofaringe parcialmente. Es inmediatamente evaluado por el servicio de Otorrinolaringología y con una pinza para cuerpo extraño se extrajo un pescado de 9,5 cm. x 6,5 cm., presentando sangrado moderado posterior a la extracción, además mucosa faringea hiperémica y epiglotis con leve edema que no obstruye el espacio glótico. Se realiza radiografía de tórax, siendo normal. Se le indica antibióticos, analgesia y esteroides. A las 24 horas se decide el alta por mejoría. La oportuna y rápida intervención influyó en la disminución de las complicaciones y de la mortalidad.

Introduction: In the Goajira of the state of Zulia, Venezuela, fishing is the occupation that provides alimentary mainteinance for the majority of the population. Because of the rarity of the case, we report a 14 year old male who was brought to the emergency room of the University Hospital of Maracaibo, because of an accidental ingestion of a live fish, while he was fishing with a net. The fish jumped into the patient’s mouth causing nausea, anguish, difficulty to breathe, excessive salivation, erythematous oropharynx. With a direct laryngoscopy the fish was observed in the oropharynx which was partially occluded. The patient was immediately evaluated by the department of otorhinolaryngology and a fish of 9.5 cm x 6.5 cm was extracted with a clip for foreign bodies. Moderate bleeding occurred after the extraction. The pharyngeal mucosa was hyperaemic and there was a slight edema of the epiglotis with no obstruction of the the glotic space. Chest rhadiograph was normal. The patient was treated with antibiotics, analgesia and steroids and was discharged 24 hours after admission. The opportune and quick intervention contributes to decrease the frequency of complications and the mortality associated with foreign bodies in the respiratory tract.
