Introduction: Hand hygiene is the most cost-effective intervention to mitigate Healthcare-Associated Infections (HAIs), preventing up to 50% of these cases. Objective: To analyze the scientific-technological milestones of hand hygiene through a documentary review with dialectical analysis, in order to ground the transition towards Patient Safety 4.0 under nursing leadership. Methods: A documentary review with dialectical analysis structured as a "technical-scientific Top 10". It spans from the 18th-century epistemological void to digital biosafety, using Morin's Complexity Theory to analyze the human-microorganism-infrastructure interaction. Results: The analysis prioritizes key milestones: contributions from Semmelweis (mechanical transmission), Nightingale (biostatistics), and Lister (chemical antisepsis); 80% alcohol as the gold standard and chlorhexidine for its residual effect in invasive procedures. Disposable paper is critical for removing debris and preventing mechanical transfer; intelligent systems (IoT/AI) overcome observer bias, capturing 100% of opportunities compared to 5% in traditional surveillance. Conclusion: Hand hygiene is a high-complexity praxis that integrates historical awareness and technological precision. The transition to cyber-physical systems allows nursing to lead proactive risk management, guaranteeing safety as a human right.
