Abstract

Introduction: In Venezuela, a South American low-income country, the demographic and epidemiological profile is changing. The number of older adults with multimorbidity and fragility is rapidly increasing. Methods: A descriptive, analytical, cross-sectional study was conducted during February–July 2025 to evaluate the frequency and clinicopathological characteristics of patients admitted to the Internal Medicine service who met polypathological criteria at a tertiary hospital in Caracas, Venezuela. Results: Of 978 patients admitted, 79 (8.08 %) met the criteria. Older adults (mean age 66.2 ± 11 years), overwheight/obese (57.5 %) and females (54.4%) predominated. In-hospital mortality was 17.72%. The average number of polypathology categories per patient was 3.58. The most frequent categories, relative to the total of categories of polypathology, were: ischemic heart disease (17.68 %), chronic kidney disease (16.96 %), and heart failure (16.25 %). Infections were the primary reason for admission, 46.84%. A highly significant association was found between polypathology and polypharmacy (p=0.002). Conclusion: polypathology in this cohort showed a lower in-hospital prevalence than reported in the literature, occurred mainly in older adult females, with a high average number of categories per patient, and was related to cardiovascular diseases. Admissions were most frequently due to infectious decompensation, and polypathology was significantly associated with polypharmacy.
