Clinical, imagenological and histopathological correlation in mammary gland malignant tumors. Experience at the Dr. Francisco Antonio Risquez hospital (2004-2014)

Abstract

Introduction: Breast cancer represents the first cause of death in women in our country and is the pathology that is diagnosed more frequently in women worldwide. Objective: To determine the clinical, imaging and histopathological correlation of malignant tumors of the mammary gland in patients surgically treated in the General Surgery Department of the Dr. Francisco Antonio Risquez Hospital in the period 2004-2014. Methods: A retrospective, cross-sectional epidemiological study was conducted. All the patients who underwent surgery in our department for malignant tumors of the mammary gland, who had a complete clinical history, were included. Results: 47.83 % were patients of 41-50 years, most consulted for presenting palpable mass. Tumors were more frequent in the right breast, measuring 52.17 % >≥2 and <5 cm, being mobile in 65.21 % of the cases and non-painful in 30.43 %. The 52.17 % of mammograms were BI-RADS 0, only 5 cases BI-RADS 4 and 2 cases BI-RADS 5. Most of the ecosonograms reported mammary tumor. It was observed that 52.17 % of PAAF (Puncture with Fine Needle Aspiration), reported ductal carcinoma. Partial mastectomy and modified radical were used as therapeutic options. The 92.30 % of the cases turned out to be ductal carcinoma in the definitive biopsy. There is an association between PAAF, the ecosonogram, mammography and definitive biopsy, with mammography being the weakest relation, in this study the sensitivity of PAAF was 86.96 %, that of the ecosonogram 82.61 % and mammography 30.43 % The specificity for the three tests was 100 %. Conclusions: For the diagnosis of malignant tumors of the mammary gland, it is fundamental to correlate the clinical, imaging and histopathological studies and in this way increase their sensitivity and specificity, making detections in early stages, improving prognosis and morbidity.-mortality.
