Crecimiento Fetal Restringido: Valoración Ecocardiográfica del Llenado Ventricular Mediante el Índice E/A

Abstract

Objetivo: Evaluar las características del llenado ventricular cardíaco fetal mediante determinación del índice E/A en un grupo de gestantes con crecimiento fetal restringido (CFR) que acudieron al Servicio de Medicina Materno Fetal del Hospital Maternidad Concepción Palacios, entre enero y octubre 2022. Métodos: Estudio comparativo prospectivo, descriptivo, transversal, de una muestra no probabilística de 86 gestantes divididas en dos grupos comparables estadísticamente, uno con CFR y otro por gestantes sanas que constituyeron el grupo control. Se determinó el índice E/A entre las semanas 24 y 42. Se realizó un análisis univariante, con medidas de tendencia central y de dispersión de datos (Media y desviación estándar) para aquellas variables continuas; mientras que para las variables categóricas se utilizó el porcentaje y la distribución percentilar. Se compararon estadísticamente las diferentes variables con las pruebas de T de Student, U de MannWhitney, ANOVA de una vía, Chi cuadrado de Pearson para un nivel de significancia del p<0,05. Resultados: En gestaciones mayores a 32 semanas, el índice E/A del ventrículo derecho fue significativamente menor en los casos de CFR respecto al control (p=0,009) mientras que, para menores a 32 semanas, se encontró una diferencia significativamente mayor en el índice E/A del ventrículo izquierdo en los casos con CFR moderado o grave, en relación al control. En el análisis del índice de pulsatilidad promedio de la arteria uterina para ambos grupos, se encontró una diferencia significativa mayor en el índice E/A del ventrículo izquierdo, solo para aquellas gestaciones con índice de pulsatilidad elevado o patológico. Objective: To evaluate the characteristics of fetal cardiac ventricular filling by determining the E/A ratio in a group of pregnant women with restricted fetal growth (CFR) who attended the Maternal Fetal Medicine Service of the Concepción Palacios Maternity Hospital, between January and October 2022. Methods: Prospective, descriptive, cross-sectional comparative study of a non-probabilistic sample of 86 pregnant women divided into two statistically comparable groups, one with CFR and the other with healthy pregnant women who constituted the control group. The E/A index was determined between weeks 24 and 42. A univariate analysis was performed, with measures of central tendency and data dispersion (Mean and standard deviation) for those continuous variables; while for categorical variables the percentage and percentile distribution were used. The different variables were statistically compared with Student's T tests, Mann-Whitney U, one-way ANOVA, and Pearson's Chi square for a significance level of p<0.05. Results: In gestations greater than 32 weeks, the E/A index of the right ventricle was significantly lower in CFR cases compared to the control (p=0.009) while, for less than 32 weeks, a significantly greater difference was found in the E/A ratio of the left ventricle in cases with moderate or severe CFR, in relation to the control. In the analysis of the average pulsatility index of the uterine artery for both groups, a significantly greater difference was found in the E/A index of the left ventricle, only for those pregnancies with a high or pathological pulsatility index
