Objective: To determine the risk factors associated with the appearance of subcapsular hepatic hematoma in patients with hypertensive disorders of pregnancy that entered in Maternal and Fetal Medicine Service of Concepcion Palacios Maternity in the period between January 2012 and December 2016. Methods: Retrospective study of cases and controls. The study population made up of a group of 2136 patients with preeclampsia/eclampsia. The sample was non – probabilistic consisting of 24 patients divided into two groups: the first of patients with subcapsular hepatic hematoma (6 cases), and the second without subcapsular hepatic hematoma (18 controls). The clinical histories of this patients were reviewed for data collection. Results: Half of the patients were white and primiparous, in both groups. In the group with HSC, 4 patients (67%) had severe preeclampsia, and 2 (33%) had eclampsia. In the group without HSH, 16 (89%) had severe preeclampsia, and 2 (11%) had eclampsia. All of them attended prenatal care and the majority without HSH attended more than 5 consultations. All the patients with HSH presented HELLP syndrome. The mean TGO was 186.6 and that of TGP was 205 mg / dl in the group with HSC, while in the patients without HSH 53.2 and 46.4 mg / dl respectively. Conclusions: HELLP syndrome, marked elevation of transaminases and inadequate prenatal control are risk factors for HSH. There is no relationship between age, parity and race, and the risk of developing HSH
