Gastric adenocarcinoma extremely well differentiated with cylindrical cells: case report

Abstract

The extremely well differentiated gastric adenocarcinoma corresponds to an entity initially described in 1984, the epithelium of the gastric mucosa where the finding corresponds to a histologically benign entity but with an aggressive behavior. Subsequently, Pelayo Correa described in carcinogenesis cascade of gastric cancer the "point of no return" the onset of atrophic gastritis and intestinal metaplasia, the latter conventionally type described small intestine and colon, as precursor lesions for the development of adenocarcinoma intestinal type gastric In the case presented, we found an 80-year-old female patient with a history of hypertension and hypertensive heart disease in the dilated phase, with symptomatology of melenic evacuations, early satiety, asthenia, adynamia and weight loss. Radiological and endoscopic studies are performed where they report high lesion, large (10cm), located in the fundus, with initial report of Borrmann I gastric cancer, however, later explorations demonstrated the non-invasion of the submucosal lesion (lifting test positive) and histopathological findings allowed endoscopic mucosal resection of the same in "piece meal" satisfactorily in multiple sessions, with total improvement of symptoms. A brief bibliographic review of the subject is made in the presence of rare histological presentation of ciliated metaplasia in an early gastric lesión.
