Doppler en arterias hepática y cística: un apoyo en el diagnostico ultrasonográfico de la colecistitis aguda. Revisión narrativa

Abstract

Mundialmente, la colecistitis aguda es una enfermedad de importante incidencia cuyas complicaciones representan tanto un riesgo para la vida del paciente, como un alto costo para el sistema sanitario. El papel del ultrasonido es indudable como estudio de primera línea en el diagnóstico y manejo de la patología. En casos donde los criterios clásicos para calificación de una colecistitis aguda sean dudosos, o haya discordancia en la correlación clínico-sonografica, las velocidades sistólicas máximas de la arteria cística y de la arteria hepática pueden ser la solución para dirimir dichas diferencias y realizar la adecuada caracterización de la enfermedad para su adecuada tipificación y manejo correspondiente. El enfoque de esta revisión narrativa es describir la técnica para valorar las arterias hepática y cística en pacientes con cuadros de colecistitis aguda y mencionar la validez estadística de dichos elementos, para incorporarla a la práctica y reforzar el reconocimiento de la entidad. Acute cholecystitis is a disease with significant incidence worldwide, with associated complications that represent both a risk to the patient’s life and a high cost to the healthcare system. The role of ultrasound is unquestionable today as a first-line study in the diagnosis and management of this condition. However, in cases where the classic criteria for classifying acute cholecystitis are unclear, or there is a dispute due to discrepancy in the clinical-sonographic correlation, peak systolic velocities of both the cystic and hepatic arteries may be the solution to resolve these differences and adequately characterize the pathology for proper typing and management. Therefore, the focus of this narrative review is to describe the technique for assessing the hepatic and cystic arteries in patients with acute cholecystitis and, in turn, to discuss the statistical validity of these elements, with the goal of incorporating them into clinical practice to reinforce recognition of this entity.
