Histoplasmosis crónica diseminada en paciente inmunosuprimida con presentación clínica centrofacial.
Abstract
Histoplasmosis is a deep mycosis with a high incidence in tropical countries of South America. It is more common in men and immunocompromised. A case of a 25-year-old woman is reported, who presented a papule in the left nostril, which evolved into an ulcerated tumor and perforation of the nasal septum, over the course of one year. During this time, she went to various specialists, without improvement, which is the reason that she came to our center. Skin biopsy: granulomatous infiltrate with vacuolated macrophages full of encapsulated spores, PAS positive. Paraclinical: pancytopenia, positive serology for HIV, chest tomography with bilateral cavitated nodules, skin tissue smear and bone marrow biopsy with intracellular blastoconidias, concluding the diagnosis of chronic disseminated histoplasmosis. The centrofacial clinical presentation with perforation of the nasal septum is unusual and infrequent, with few cases described in the literature.
