The 5S methodology, originating in the Toyota Production System, has been extrapolated to the healthcare system as a tool to improve the organization, efficiency, and quality of services. This article reviews its theoretical applicability in a general surgery service, considering both the clinical practice aspect (clinical care) and the academic aspect (training of undergraduate and postgraduate students). Methods: A systematic review of sources published between 2020 and 2025 on Lean 5S in the healthcare field, focusing on surgical services, was conducted. Conceptual examples of 5S implementation in surgical areas were analyzed, such as instrument organization, operating room times, and teaching activities. Results: The evidence indicates that this tool allows for improved space organization and workflows, as well as reduced waste and delays in service, increasing team satisfaction. Conclusion: The 5S methodology has great potential to optimize processes in a general surgery service and thus foster a culture of continuous improvement. Its implementation can result in more organized, safer, and more efficient operating rooms, while its integration into academia trains future professionals in quality management. However, institutional leadership and sustained monitoring are required to maintain these benefits.
