Hernioplastia inguinal. eficacia analgesica postoperatoria intratecal: bupivacaina 0,5 % - sulfato de magnesio 75 mg vs bupivacaina 0,5 % - sulfato de magnesio 100 MG

Abstract

Objetivo: Comparar la eficacia analgésica postoperatoria intratecal de bupivacaína 0,5 % - sulfato de magnesio 75 mg vs bupivacaína 0,5 % - sulfato de magnesio 100 mg en hernioplastía inguinal. Métodos: Estudio comparativo, prospectivo y aleatorio. Muestra: 25 pacientes de ambos sexos, mayores de 18 años ASA I – II, divididos en Grupo A (100 mg) y Grupo B (75 mg) del Servicio de Cirugía III, entre enero - junio 2018. Se midieron variables hemodinámicas: frecuencia cardíaca, presión arterial sistólica, diastólica, media y saturación de oxígeno basal y post técnica neuroaxial. En Sala de Cuidados Postanestésicos se evaluó la analgesia postoperatoria hasta por 12 horas. Resultados: No hubo diferencias estadísticamente significativas en el análisis intergrupal ni intragrupal en las variables hemodinámicas. En cuanto al grado de dolor postoperatorio no se apreciaron diferencias estadísticamente significativas entre ambos grupos de estudio. Intragrupalmente, al comparar los resultados obtenidos al momento del ingreso a Sala de Cuidados Postanestésicos con los minutos 15, 30, 45 y 60, y posteriormente a las horas 2, 4, 6, 8, 10 y 12 de su ingreso, no se detectaron diferencias estadísticas importantes. Conclusión: El sulfato de magnesio a ambas dosis (100 mg y 75 mg) por vía espinal resultó igual de eficaz para analgesia postoperatoria (hernioplastía inguinal), mostrando un perfil más favorable a dosis coadyuvante de 100 mg en cuanto a menor grado de dolor, requerimientos de morfina postoperatoria y tiempo de rescate con la misma. Objective: To compare the intrathecal postoperative analgesic efficacy of bupivacaine 0.5% - magnesium sulfate 75 mg vs bupivacaine 0.5% - magnesium sulfate 100 mg in inguinal hernioplasty. Hemodynamic variables were measured: heart rate, systolic blood pressure, diastolic, mean and oxygen saturation, baseline and post neuraxial techniques. In the Post-Anesthesia Care Room, postoperative analgesia was evaluated up to 12 hours. Results: There were no statistically significant differences in the intergroup or intragroup analysis in the hemodynamic variables. Regarding the degree of postoperative pain, no statistically significant differences were observed between the two study groups. Intragrupally, when comparing the results obtained at the time of admission to Post-Anesthesia Care Room with minutes 15, 30, 45 and 60, and after hours 2, 4, 6, 8, 10 and 12 of their admission, no significant statistical differences were detected. Conclusion: Magnesium sulphate at both doses (100 mg and 75 mg) by the spinal route was equally effective for postoperative analgesia (inguinal hernioplasty), showing a more favorable profile at a dose of 100 mg adjuvant in terms of less pain, postoperative morphine requirements and rescue time with it.
