dc.description.abstractObjetivos: Evaluar dolor postoperatorio en pacientes sometidas a histerectomía abdominal, comparando anestesia general con propofol- remifentanilo versus sevoflurane-remifentanilo utilizando la ESCALA VISUAL ANÁLOGA (EVA); además se evaluó el consumo postoperatorio de morfina en ambos grupos de estudio durante las primeras 24 horas. Métodos: En este estudio prospectivo, comparativo, simple ciego y aleatorizado, 48 pacientes femeninas, con edades comprendidas entre 21-60 años, con estatus físico ASA I-II, sometidas a histerectomías abdominales de manera electiva, fueron asignadas al azar a 2 grupos de estudio; sevoflurane-remifentanilo (SR) y propofol-remifentanilo (PR) para el mantenimiento de la anestesia. Resultados: Las pacientes que recibieron mantenimiento anestésico con sevoflurane-remifentanilo presentaron medianas de EVA mayores dentro de la primera hora del postoperatorio, en comparación con el grupo que recibió propofol-remifentanilo, siendo ésta diferencia estadísticamente significativa. (P < 0,05). Durante la primera hora, 25% de las pacientes requirieron de morfina siendo más frecuente en las pacientes que recibieron sevoflurano, encontrándose una asociación estadísticamente significativa entre el uso de morfina y el fármaco recibido (X2=5,44; 1 gl; P valor= 0,0173 < 0,05). Conclusión: El mantenimiento anestésico con propofol en pacientes sometidas a histerectomía abdominal abierta, podría tener efectos analgésicos y antihiperalgésicos; sin embargo éstos efectos desaparecen tan pronto la concentración del fármaco disminuye. Objectives: Evaluate postoperative pain in patients undergoing abdominal hysterectomy, comparing general anesthesia with propofol-remifentanyl versus sevoflorane-remifentanyl using the VISUAL ANALOG SCALE (VAS); moreover, the postoperative consumption of morphine will be assessed in both study groups during the first 24 hours. Methods: In this prospective, comparative, single-blind, randomized study, 48 female patients, aged between 21 and 60 years, physical status ASA I-II undergoing abdominal hysterectomy electively, were randomly assigned to two groups of study; sevoflorane-remifentanyl (SR) and propofol-remifentanyl (PR) for the maintenance of anesthesia. Results: Patients who received anesthetic maintenance with sevoflurane-remifentanil had a greater median VAS within the first hour of the postoperative period, compared to the group receiving propofol-remifentanil, which were statistically significant difference. (P <0.05). In the first hour, 25% of the patients required morphine being more frequent in the patients who received sevoflurane, demostrating a statistically significant association between the use of morphine and the drug received (X2 = 5.44; 1 gl; P Value = 0.0173 <0.05). Conclusion: anesthetic maintenance with propofol in patients submitted to open abdominal hysterectomy, could have analgesic and antihyperalgesic effects; however these effects disappear as soon as the drug concentration decreaseen_US
