| dc.description.abstract | Comparar el confort del cirujano durante la colecistectomía laparoscópica con la técnica americana vs. la técnica modificada. Métodos: Se realizó un estudio prospectivo, descriptivo, comparativo y de corte transversal en pacientes con litiasis vesicular. Servicio de Cirugía General (I, II, III, IV). Hospital Universitario de Caracas, período enero-agosto 2022. Resultados: Se realizaron 77 colecistectomías laparoscópicas, de las cuales la técnica americana fueron 32 (41,6%) y la técnica modificada con 45 (58,4%). Los residentes de tercer año duraron de 30 a 60 minutos con la técnica modificada (70,7%) frente a la técnica americana (16,7%), siendo esta diferencia estadísticamente significativa (p˂0,0001). Con la técnica americana se evidenció como zona dolorosa la muñeca izquierda (62,5%), por otro lado, en la técnica modificada reportan en 91,1% que no tienen zona dolorosa, con diferencias estadísticamente significativas (p˂0,0001). El 43,8% de los cirujanos no se sienten cómodos con la técnica americana, mientras que en la modificada el 97,8% reportaron la técnica cómoda siendo esta diferencia significativa (p ˂0,0001). Los cirujanos refirieron un esfuerzo difícil en el 56,3% con la técnica americana mientras que con la modificada el 82,2% de los cirujanos realizaron un esfuerzo leve para desarrollar la técnica. el grado de dolor en las extremidades fue mayor en los que realizaron la técnica americana que lo que realizaron la Modificada (p ˂0,0001). Results: 77 laparoscopic cholecystectomies were performed, of which the American technique was 32 (41.6%) and the modified technique was 45 (58.4%).
