dc.description.abstractEstablecer la influencia de los factores pronósticos clínico-patológicos en la evolución de pacientes con diagnóstico de cáncer de cuello uterino en estadios tempranos, tratadas con histerectomía radical en el Instituto de Oncología Dr. Luis Razetti de Caracas entre los años 2005 – 2013. Métodos: Estudio retrospectivo, descriptivo, transversal, La población estuvo conformada por todas las pacientes con cáncer de cuello uterino estadio temprano, sometidas a histerectomía radical y linfadenectomía pelviana de manera electiva,. La muestra estuvo constituida por 143 pacientes que cumplieron con los s criterios de inclusión.Resultados: En un periodo de 8 años (2005 – 2013) comprendieron 143 pacientes. La edad promedio de la muestra fue de 43,17 años El tipo histológico más frecuente fue el carcinoma epidermoide en 92 pacientes (64,34 %), adenocarcinoma 23pacientes 16. 08 %, del tamaño tumoral se registraron tres grupos Tamaño tumoral < 2 en 55 pacientes 38,46%, 2,1-4 cms en 43 pacientes 30,07, > 4cms en 7 pacientes 4,90 En el 43% de las muestras se identificó la penetración profunda del estroma (mayor de un tercio) no se halló la misma mientras que un 56 % este factor no fue identificado. La afectación parametrial resulto ausente en 56 % de las pacientes y presente en 43 % restante.cuanto a la afectación vaginal encontramos ausencia de la misma en 96 % y presencia de enfermedad vaginal en solo el 4 % de la serie. en las disecciones izquierdas se obtuvieron entre 7 a 10 ganglios un 16.08 % solo en un 5.6 % más de 10 ganglios disecados; mientras que en las disecciones derechas entre 7 a 10 ganglios en un 12.58 y más de 10 ganglios Siendo el mayor número de paciente disecciones negativas en un 96.6 % y 95.8 % respectivamente.En cuanto al tratamiento adyuvante se encontró que recibieron 114 pacientes , La mortalidad de las pacientes fue de 4,20%, representados por 6 casos.Se analizaron los factores pronósticos en aquellos pacientes con recaída, destacándose el estadio IB1 como el más frecuente para esta variable, presente en el 87,5% de los casos (Riesgo Atribuible RA 0,23, Fae 0,56, p <0,05) en menor porcentaje el estadio IIB, solo el 12,5%, la penetración estromal en 50% de los casos (RA 0,47%, Fae 0,75%, p <0,05), la extensión a parametrios solo en 12,5%, extensión vaginal 12,5%, sin positividad de ganglios en algún paciente. conclusion El tipo histológico con mayor incidencia fue el carcinoma epidermoide, se puede asociar con la infección por VPH La invasión profunda del estroma es un factor pronóstico importante para las recaídasEl índice de mortalidad es bajo y se asocia a mayores estadios de la enfermedad, asi mismo se evidencio que los pacientes que la recaida en pacientes no estuvo asociada a el tipo histológico si no al estadioen_US

The population consisted of all patients with early stage cervical cancer, who underwent elective radical hysterectomy and pelvic lymphadenectomy. Regarding vaginal involvement, we found its absence in 96% and the presence of vaginal disease in only 4% of the series. in the left dissections between 7 to 10 lymph nodes were obtained in 16.08% only in 5.6% more than 10 dissected lymph nodes; while in the right dissections between 7 to 10 lymph nodes in 12.58 and more than 10 lymph nodes, the largest number of patients being negative dissections in 96.6% and 95.8% respectively. Regarding adjuvant treatment, it was found that 114 patients received, mortality of patients was 4.20%, represented by 6 cases. Prognostic factors were analyzed in those patients with relapse, highlighting stage IB1 as the most frequent for this variable, present in 87.5% of cases (Risk Attributable RA 0.23, Fae 0.56, p <0.05) stage IIB in a lower percentage, only 12.5%, stromal penetration in 50% of cases (RA 0.47%, Fae 0, 75%, p <0.05), extension to parametria only in 12.5%, vaginal extension 12.5%, no positive nodes in any patient. conclusion The histological type with the highest incidence was squamous cell carcinoma, it can be associated with HPV infection The deep invasion of the stroma is an important prognostic factor for relapses The mortality rate is low and is associated with higher stages of the disease, likewise It was evidenced that the patients that the relapse in patients was not associated with the histological type but with the stageen_US
