| dc.description | To know clinical and anatomopathological prognostic factors in renal cell cancer patients treated at the IVSS Hospital Oncology Service January 2006 - January 2016. Materials and Methods: We retrospectively reviewed the medical histories of patients who met inclusion criteria. Clinical and anatomopathological characteristics were analyzed. A multivariate study of survival variables and Kaplan-meier curves were performed. Results: 142 patients evaluated, mean age 54 years, male 59.9%. Symptoms in 81.7%, mainly low back pain and urine bloody. Hemoglobin average 12.6gr / dl. ECOG 0 87.3%. Tumor size> 7 cm 54.8%. Radical nephrectomy 94.4%. Clear cell histology 76.8%. Furhman II nuclear grade 33.9%. Lymphovascular invasion present 20.4%. Tumor necrosis present 23.2%. Main areas of lung, bone and liver metastasis. Median relapse-free interval 27 months. Median overall survival 33 months. Low back pain (p <0.013), T2, T3a and T4 (p <0.002), and TNM (p <0.001) proved to be unfavorable prognostic factors for relapse. In a multivariate analysis using the Cox model, performance status (p <0.03), lymph node invasion (p <0.002), lymphovascular invasion (p <0.008), necrosis (p <0.001) and presence of metastases (p <0.001), Independent prognostic factors were associated with mortality. At the end of the study, 73.6% of the patients were alive and 26.4% had died. Conclusions: We established prognostic factors in our population ECOG ≥ 2, nodal involvement, metastasis, angiolymphatic invasion and tumor necrosis. | dc.description.abstract | Conocer factores pronósticos clínicos y anatomopatológicos en los pacientes con cáncer de células renales atendidos en el Servicio Oncológico Hospitalario del IVSS enero 2006 - enero 2016. Materiales y Métodos: Retrospectivamente se revisaron historias clínicas de pacientes que cumplieron criterios de inclusión. Se analizaron características clínicas y anatomopatològicas. Se realizó estudio multivariado de variables para sobrevida y confeccionaron curvas Kaplan-meier. Resultados: 142 pacientes fueron evaluados, edad promedio 54 años, masculinos 59.9%. Síntomas en 81.7%, principalmente dolor lumbar y hematuria. Hemoglobina promedio 12.6gr/dl. ECOG 0 87.3%. Tamaño tumoral >7 cm 54.8%. Nefrectomía radical 94.4%. Histología células claras 76.8%. Grado nuclear Furhman II 33.9%. Invasión linfovascular presente 20.4%. Necrosis tumoral presente 23.2%. Principales zonas de metástasis pulmón, óseo e hígado. Mediana de intervalo libre de recaída 27 meses. Mediana de sobrevida global 33 meses. Dolor lumbar (p <0.013), T2, T3a y T4 (p <0.002), y TNM (p <0.001) resultaron ser factores pronostico desfavorables para recaída. En análisis multivariado aplicando modelo de Cox, el estado de desempeño (p <0.03), la invasión ganglionar (p <0.002), invasión linfovascular (p<0.008), necrosis (p <0.001) y presencia de metástasis (p <0.001), resultaron factores pronósticos independientes asociados a mortalidad. Al final del estudio 73.6% de los pacientes estaban vivos y 26.4% habían fallecido. Conclusiones: Se estableció como factores pronósticos en nuestra población ECOG ≥ 2, afectación ganglionar, metástasis, invasión angiolinfática y necrosis tumoral, se recomienda reportes completos de anatomía patológica, realizar disección ganglionar, identificar pacientes de alto riesgo, con el fin de brindar terapéutica y seguimiento oportuno. | en_US |
