Cáncer de cuello uterino asociado al embarazo

Resumen

La asociación entre cáncer y embarazo es una situación rara, pero su frecuencia ha aumentado recientemente.El cáncer cervical y sus precursores se diagnostican con frecuencia durante el embarazo y gracias a la pesquisacitológica obligatoria durante la consulta prenatal, la enfermedad se diagnostica en estadios tempranos. El manejoadecuado requiere que se establezca un balance entre el bienestar y la sobrevida del feto y la seguridad oncológica dela madre. Las conductas de diagnóstico y tratamiento se han modificado con el pasar de los años, desde una posturaintervencionista, con tendencia a interrumpir el embarazo, a una conducta más conservadora, para permitir la madurezfetal e, incluso conservar la fertilidad futura. En la presente revisión se presenta información epidemiológica, pautasde diagnóstico y tratamiento de lesiones preinvasivas y de cáncer cervical por estadios, alternativas terapéuticaspreservadoras de fertilidad, recomendaciones sobre la vía de parto y pronóstico materno fetal.

The association between cancer and pregnancy is a rare situation, but their frequency has increased recently. Cervicalcancer and its precursors are frequently diagnosed during pregnancy, and thanks to the mandatory cytological researchduring the prenatal consultation, the disease is diagnosed in the early stages. Proper management requires establishing abalance between the well-being and survival of the fetus and the oncological safety of the mother. Diagnosis and treatmentbehaviors have changed over the years, from an interventionist posture, with a tendency to interrupt pregnancy, to amore conservative behavior, to allow fetal maturity, and even to preserve future fertility. In this review, epidemiologicalinformation, diagnostic guidelines and treatment of pre-invasive lesions and cervical cancer by stages, fertility-preservingtherapeutic alternatives, recommendations on the route of delivery and maternal fetal prognosis, are presented.
