Approximately 70-90% of the cases of disseminated histoplasmosis present with gastrointestinal involvement, predominantly in immunocompromised patients. The present case deals with a 40 year old male patient, who denies comorbidities and refers to a current illness of 06 months of evolution, characterized by abdominal pain, located in the mesogastrium, of moderate intensity, associated with liquid stools with blood and weight loss. Endoscopically in the cecum, a raised, nodular, friable lesion is observed on biopsy, whose pathology report indicates the presence of Histoplasma Capsulatum; Given these findings, HIV serology was performed, which yielded a positive result, and a chest X-ray, with no abnormal findings, for which reason medical treatment with itraconazole was indicated, obtaining a satisfactory clinical evolution.
