VENTILACIÓN MECÁNICA PROTECTIVA PERIOPERATORIA

RESUMEN: Cada paciente sometido a ventilación mecánica durante el acto operatorio, tiene el riesgo potencial de padecer lesión pulmonar, desde la ultraestructura hasta el propio funcionamiento del mismo; lo cual genera complicaciones respiratorias desde el postoperatorio inmediato hasta las siguientes 72 horas posteriores a la intervención quirúrgica. Estos mecanismos, han sido estudiados y tienen bases fisiopatológicas bien reconocidas. Para contrarestar estos procesos deletéreos; se han estudiado varias estrategias que en su conjunto se reconocen como Ventilación Mecánica Protectiva Perioperatoria; que aplicadas en el transoperatorio podrían atenuar los efectos negativos sobre el aparato respiratorio mientras el pulmón sea mecánicamente ventilado.

ABSTRACT: Each patient subjected to mechanical ventilation during the operative act, has the potential risk of suffering lung injury, from ultrastructure to its own functioning; which generates respiratory complications from the immediate postoperative period until the next 72 hours after the surgical intervention. These mechanisms have been studied and have well-recognized pathophysiological bases. To counteract these deleterious processes; several strategies have been studied that, as a whole, are recognized as Perioperative Protective Mechanical Ventilation; that applied in the transoperatory could attenuate the negative effects on the respiratory system while the lung is mechanically ventilated.
