Intramucosal gastric adenocarcinoma in the colon originating from a gastric heterotopia: case report and review of the literature

Abstract

Gastric heterotopia is defined as the presence of ectopic tissue outside its normal location, which can be found at various levels of the gastrointestinal tract, including the biliary tract and pancreas. The risk of adenocarcinoma is not well established due to the unusual nature of this lesion. The present case concerns a 54-year-old female patient, who reports long-standing abdominal pain located in the left flank, which has worsened in recent weeks. A colonoscopy was performed in which a sessile polyp, approximately 1 cm in diameter, with a smooth surface, without vascular or glandular changes, was observed in the descending colon and was resected with a cold snare. This polyp is reported by pathological anatomy as a pyloric-type gastric mucosa with epithelial malignant neoplastic area whose immunohistochemistry markers were positive for CK7, CDX2, AE1/AE3 and Ki67, concluding then as Intramucosal gastric adenocarcinoma originating from polylipoid gastric heterotopia of the descending colon. Currently the patient is asymptomatic.
