Hepatitis alcohólica: pautas mundiales

La Organización Mundial de la Salud en el año 2014 indicó que el uso de alcohol es responsable de 3.3 millones de muertes cada año, lo cual representa un 5.9% por todas las causes de muerte. En nuestro país, el consumo de alcohol representa la primera causa de enfermedad hepática crónica, desplazada en otros países por Virus de la Hepatitis B-C o por etiología Metabólica. Un consumo diario de alcohol de 40 g. para el hombre y de 20 o más gramos para la mujer, durante unos 7 a 10 años, constituyen la dosis de riesgo para el desarrollo de Enfermedad Hepática Crónica, Fibrosis Hepática, Cirrosis y Cáncer de Hígado. La ictericia de reciente comienzo en pacientes con consumo excesivo de alcohol debe llevar al clínico a sospechar una hepatitis alcohólica. La incidencia de este síndrome clínico permanece desconocida, pero se sabe que se ha incrementado y continua siendo más frecuente en hombres aunque el sexo femenino es un factor de riesgo. En su manejo es muy importante establecer el grado de severidad. Existen diferentes scores para identificar las formas severas de HA, así como el riesgo de mortalidad. La Hepatitis Alcohólica puede formar parte de la evolución, tratándose de pacientes en su mayoría cirróticos con alta probabilidad de morir, especialmente cuando desarrollan una condición severa. A pesar de que el uso de esteroides sigue siendo controversial la identificación temprana de no respondedores permite ser utilizado evitando exposición innecesaria. El uso de acetilcysteina puede combinarse en pacientes con HA severa. En caso de no respondedores debe ser considerado el trasplante hepático.
