Pautas para la esofagitis cáustica

La esofagitis cáustica es un problema de salud pública que afecta tanto a niños como adultos, representa una morbilidad elevada por complicaciones a largo plazo como la estenosis esofágica y puede poner el peligro la vida del paciente cuando la lesión esofágica o gástrica es severa u ocurre la perforación del órgano afectado. La presencia de tres o más síntomas como sialorrea, disfagia y lesiones en orofaringe se asocia a lesiones más graves en esófago. La escala de Drool fue desarrollada para predecir con los síntomas la severidad de las lesiones. La endoscopia digestiva alta es la prueba diagnóstica estándar evaluar lesiones esofágicas y gástricas, así como para predecir el pronóstico y establecer un plan de manejo, según la clasificación de Zargar. Las pruebas de laboratorio, radiología, tomografía y ecoendoscopia son pruebas que contribuyen con la evaluación integral del paciente, en especial cuando se presentan complicaciones como sepsis, perforación esofágica, o existe contraindicación de la endoscopia. El tratamiento comprende la estabilización hemodinámica del paciente, asistencia respiratoria, inhibición de la secreción ácida y administración de antibióticos o esteroides, dependiendo de la severidad de la lesión. El inicio de la dieta dependerá del grado de lesión y puede ir desde líquida a blanda o nutrición parenteral. La Estenosis esofágica es la complicación más frecuente, requiere tratamiento de dilataciones esofágicas y la perforación esofágica la consecuencia más tenida, cuyo tratamiento es usualmente conservador.
