PANUVEITIS SIFILÍTICA, NEUROSÍFILIS Y SIDA EN UN PACIENTE CON PRUEBAS SEROLÓGICAS INICIALMENTE NEGATIVAS. A PROPÓSITO DE UN CASO

Uveítis es un término general que hace referencia a la inflamación del tracto uveal. En el 70 % de los casos es posible establecer
una relación causal entre diferentes tipos de uveítis y enfermedades locales y/o sistémicas. El 30 % restante de los casos no es posible
asociarlos con enfermedades primarias oculares o sistémicas. Se presenta el caso clínico de un paciente masculino de 52 años de edad, quien
consultó por disminución progresiva de la agudeza visual en ambos ojos, de 3 meses de evolución. En la evaluación oftalmológica se planteó
el diagnóstico de panuveítis bilateral en estudio. Se realizaron pruebas serológicas (VDRL, FTA-abs y virus de inmunodeficiencia humana),
que resultaron negativas. Luego de 21 días de tratamiento, y en vista de la disociación entre la clínica y el laboratorio, se hospitalizó al
paciente y se realizó nuevo VDRL sérico y en líquido cefalorraquídeo ambos reactivos, y el Western Blot para virus de inmunodeficiencia
humana positivo, se diagnosticó panuveítis sifilítica bilateral, neurosífilis y síndrome de inmunodeficiencia adquirida. Se administró
penicilina cristalina (24 000 000 UI/día) durante 21 días, evolucionando satisfactoriamente, con recuperación visual tot
ABSTRACT: Uveitis is a general term who makes reference to the inflammation of the uveal tract. In 70 % of the cases is possible to stablish
a causal relation between different types of uveitis and local and/or systemic disease. In the 30 % rest of cases, is not possible to associate
with primary ocular or systemic disease. We present the case of a 52 years old male patient who consults for visual acuity decrease in both
eyes, 3 months of evolution. In the first ophthalmic evaluation we considered it bilateral panuveitis in study. Serological test were made
including VDRL, FTA-abs and HIV, negatives. After 21 days of treatment, in view of the dissociation between clinic and laboratory, the patient
was hospitalized and new serological and cerebral spinal fluid VDRL was reactives, and HIV Western Blot was positive. The diagnosis was
syphilitic bilateral panuveitis, neurosyphilis and adquired immunodeficiency syndrome. Cristallin Penicillin was administered (24.000.000
IU/day) for 21 days, with a satisfactory evolution, and visual acuity was recovered.
