Título : Velicimetría doppler de la Arteria Cerebral Média: Relación cpn la asfixia perinatal del neonato de gestantes con trastornos hipertensivos del embarazo

Resumen : Objetivo: Analizar la utilidad de la velocimetría Doppler de la Arteria Cerebral Media Fetal (VDACM) como marcador de asfixia perinatal en neonatos de gestantes con preeclampsia grave. Método: Se trata de un estudio descriptivo, prospectivo, correlativo y operacional. Se incluyeron las pacientes con embarazo de 28 a 41 semanas que presentaban trastornos hipertensivos (*), atendidas en la Maternidad “Concepción Palacios” durante el período comprendido entre el 1° de marzo y el 30 de septiembre de 2014. Se valoró la condición materno fetal mediante ecografía prenatal que incluyó VDACM. Una vez obtenido el neonato se evaluaron los parámetros indicativos de bienestar fetal, incluyendo una gasometría arterial y hematología. Resultados: En 40 gestantes con preeclampsia grave (edad materna promedio 27 ± 6,58 años) se evaluaron las condiciones perinatales de los fetos. Hubo 27 neonatos (67,5 %) con asfixia perinatal, 21 con RCIU (52,5 %) y 13 con oligoamnios (32,5 %). Fallecieron 4 neonatos por asfixia perinatal severa. 22 fetos (55 %) presentaron VDACM anormal ─IP, PS o ambas─. El resultado anormal de IP en la VDACM de los fetos de gestantes preeclámpticas graves se asoció significativamente con pH de la arteria umbilical < 7,2, ingreso a la UCIN y RCIU (p = 0,0302, en los 3 casos). Conclusiones: La velocimetría Doppler de la arteria cerebral media es una prueba útil para la detección de fetos en riesgo de presentar un resultado neonatal adverso compatible con asfixia perinatal. Descripción : Objective: To analyze the utility of Doppler velocimetry of middle cerebral artery (DVMCA) as marker of perinatal asphyxia in newborns of mothers with severe preeclampsia. Method: It´s a descriptive, prospective, correlative and operational study, which included pregnant women with gestational age of 28 to 41 weeks and diagnosis of severe preeclampsia, attended at “Concepción Palacios” Maternity Hospital in the period during March 1 to September 30 of 2014. Maternofetal condition was assessment by prenatal echography procedure that included Doppler velocimetry of MCA. When get the newborn was evaluated parameters indicatives of 2 good fetal conditions including arterial gasometry and hematological analysis. Results: In 40 gestation women with severe preeclampsia (mean age 27 ± 6.58 years) were evaluated the perinatal conditions of fetuses. There were 27 newborn (67.5 %) with perinatal asphyxia, 21 with intrauterine growth retardation (52.5 %) and13 with oligohydramnios (32.5 %). Four newborn were dead by severe perinatal asphyxia. 22 fetuses (55 %) have abnormal DVMCA ─pulsatility index, systolic peak velocity or both─. The abnormal result of PI at DVMCA in fetuses of severe preeclamptic patients was statistically significant associated with pH measure of uterine artery < 7.2, admission to Neonatal Intensive Care Unit and intrauterine growth retardation (p = 0.0302, in the 3 cases). Conclusions: Doppler velocimetry of middle cerebral artery is a useful test to detect fetuses in risk of present an adverse perinatal outcome compatible with perinatal asphyxia.
